When your parent's care feels overwhelming, I'm here to help you, as your advocate partner---making sure he/she gets the right care, and that your concerns are heard.

You may also choose to do all of the work yourself, and only need guidance. I can help with this also.

Whether needed to attend in person (meet you there), by phone, online or by reviewing the after-visit summary, I will capture the important details.

HOSPITAL-to-HOME

Support when your parent--and you--need it most.

During a hospital stay or a temporary stay at a rehabilitation facility, and you cannot be there, I'm your extra set of eyes and ears---to help monitor your parent's care and safety, and keep you updated.

After discharge, I can support the safe transition to home or the next health care setting.
HOME SAFETY

I look at your parent's living space to make it safer, suggesting practical adjustments.

What is Not Provided

Medical, legal, financial, mental health, social work, therapy or hands-on care are not provided. This also means I do not diagnose medical conditions, make decisions for you, prescribe medications, or offer transportation.

My role for example, is to help you better understand health and healthcare-related information, assist with talking with a doctor on your loved ones' behalf, help to coordinate care as needed, and provide guidance as your parent (and you) make all informed decisions yourselves.
